The parent/guardian AND student must fill out and sign all electronic forms. Forms will not be accepted electronically without all blanks filled in and the electronic signature of both the parent/guardian and the student. Please type “n/a” or “none” in any blank that is not applicable. To sign the document, click inside the signature box and hold the left mouse button down. This will allow you to create an “electronic signature”. You must click on “submit” at the bottom of each form.

Click on “Proceed to Online Forms” below. The first step will be to create an account. This will allow you to print your forms at a later time if needed.

STUDENTS PARTICIPATING IN ATHLETICS. Please complete all four online forms. Also, see information below regarding medical history and physical exam.

STUDENTS PARTICPATING IN MARCHING BAND. Complete only the “Emergency Information” form and the “Drug Testing Policy/Consent Form”. Also, see information below regarding medical history and physical exam.

STUDENTS WHO DRIVE OR PARK ON CAMPUS OR PARTICIPATE IN ANY EXTRACURRICULAR CLUBS OR ORGANIZATIONS. Complete only the “Drug Testing Policy/Consent Form”.

MEDICAL HISTORY AND PHYSICAL EXAM: ATHLETES AND MARCHING BAND MEMBERS. All participants in athletics at any grade level or high school marching band are required to complete a Medical History Form. This form can be downloaded and printed from the “forms” page. ALL 2025-2026 7TH, 9TH, AND 11TH GRADE ATHLETES, 9TH AND 11TH GRADE MARCHING BAND MEMBERS, ANY FIRST YEAR ATHLETES OR ANYONE WHO ANSWERED “YES” TO ANY QUESTION 1-6 ON THE MEDICAL HISTORY FORM MUST HAVE THE PHYSICAL EVALUATION SIDE OF THE FORM COMPLETED BY A LICENSED PHYSICIAN, PHYSICIAN ASSISTANT, CHIROPRACTOR, OR NURSE PRACTITIONER. The Medical History Form and the Physical Evaluation Form (if needed), along with all online forms must be completed before participation in any UIL practices, games, performances, or matches.
